New Patient Registration

To register as a patient with Downe Health centre you can present to our reception desk to request registration forms. To make the registration process as efficient as possible it would be beneficial to bring supporting documentation with you.

The list of required documentation is as follows:

  1. Photographic ID. If moving from a GP surgery in England, Scotland or Wales this must be a passport not a Driving Licence. A birth certificate is a suitable alternative if you do not have a passport.
  2. Proof of address in the form of a utility bill/official letter.
  3. If you have never had a GP in the UK before you will also be required to provide proof of residency. This is to validate that you are entitled to NHS care.

Alternatively you can print the registration forms using the links below, returning them to the surgery with the above mentioned documentation.

You may print off a registration form and a new patient questionnaire, fill it out and bring it in with you on your first visit to the practice.

Please note that patients are registered with the practice and not an individual doctor. Patients have a right to express a preference but this may not always be possible.

Changes to Personal Details

Please inform us immediately of any change of name, address or contact numbers.
